Lindenwood  Hospice & Palliative FAQ

1.Can I keep my own Doctor ?

Yes.
• The patient can keep seeing their primary care provider (PCP).
• However, once on hospice, the hospice medical director becomes the physician of record for the terminal diagnosis.
• The PCP can:
Still be involved
Communicate with hospice
Even serve as the attending physician if the patient chooses and the PCP agrees.

Can the PCP still prescribe meds?
Yes.
• The PCP can prescribe medications.
• BUT for meds related to the terminal illness, hospice typically:
Needs to approve them
May substitute with hospice-formulary alternatives
Coordinates everything to avoid conflicts or duplication

3.  what services are included in your hospice program ?

Services may include pain and symptom management, nursing care, medications, medical equipment, counseling, spiritual support, respite care, and bereavement support for families.

5.  Does Hospice Include 24/7 Care?

Does Hospice include 24/7 care?

While some may think hospice provides 24 hours a day, 7 days a week custodial care, or full-time care at home or an outside facility, this is rarely the case. Although hospice provides a lot of support, most of the day-to-day care of a person is provided by family and friends. However, a person from a hospice care team is usually always available by phone 24/7.

2. What is Hospice ?

Hospice care is a specialized form of care designed for patients who are facing a terminal or life-limiting illness. When treatment is no longer considered an option, hospice care shifts the focus from curing the disease to managing symptoms and providing comfort to the patient.

Absolutely Your initial visit will be with the caregiver only. We look forward to meeting you so please make sure to schedule an appointment at one of our two locations.

3. What is the minimum age required to attend?

Must be 18 years or older. Private Pay, insurance, MQMD Medicaid accepted. Must have Doctors orders and diagnosis.

4. What is HCS?

The Home and community-based Services (HCS) program provides individualized services and support to people with intellectual disabilities who are living with their families.
